Deadline to register is: 

  1. Vendors, please fill out this form to secure your spot and submit your menu items along with a picture of one of your menu items to be featured on our website and promoted on social media. 
  2. Please have your temporary food service permit application completed.  You will be able to upload it with this registration form.

Menu Items

Please list up to 4 menu items and the number of tickets required.  1 ticket = $1.  

If you will have more or add to your menu later, these items may not make it onto the printed menus for the event. Example: Wings, 1 ticket each


Before Completing This Registration.

1. Complete application for a Temporary Food Service Permit, and questionnaire.  Upload completed application below.

There's an application fee of $50 payable to Mercer County Health District.

2.  Submit this registration form and send vendor fee of $90 payable to Coldwater Chamber of Commerce along with the $50 temporary food service permit application fee.
